Fee and Payment Information

Our fees vary according to the time, complexity and procedures that your appointment requires. We request that payment be made at the time of services for most situations. If circumstances prevent that, please speak to one of our staff and we will make every effort to arrange a satisfactory payment plan. Our first priority is to provide our patients with quality care — we never want finances to interfere with needed medical care. However, we do expect our patients to do their best in fulfilling their responsibility to compensate for services rendered.

Payment Methods

For your convenience, we accept cash, checks, debit and credit cards (Visa, MasterCard, Discover and American Express), and financing is available through the CareCredit® network, a healthcare financing program offered by GE Capital. For more information, please visit the CreditCare website. Any overpayment by you or your insurance company will be refunded promptly.



As a courtesy to our patients, we will file claims with your insurance carrier. At the time of your appointment, we will ask to see and make a copy of your health insurance card. This will help us in processing claims on your behalf. There may be times that we will need your cooperation in helping us file your claim, including more detailed information or previous insurance history. Refusal to participate in insurance filing or failing to provide requested additional information will be considered the same as refusing payment for services. This may result in both dismissal from the practice and the sending of your account to collections.

We do require payment of co-pays at time of service. This is an industry-wide practice and is expected at most other practices. In addition, not collecting co-pays is considered a breach of contract with most insurances and can result in stiff penalties against a medical practice.

We are currently accepting most insurance plans, including: Aetna, Cigna, Humana, United Health Care, TriCare, Medicare ProHealth and Anthem. We do not accept Medicaid patients. If you have questions about your specific insurance company or benefits available through your plan, please contact your insurance company or employer benefits representative directly.

As a patient, you have full responsibility for payment of service rendered, regardless of insurance coverage. We will be more than happy to help you with all the necessary insurance forms and answer any specific questions regarding fees and insurance. If a patient does not have insurance or if a service is not covered by insurance, we ask for a deposit prior to rendering that service. Please contact us to discuss your specific situation and we will make every effort to arrange a satisfactory payment plan. Just as fees are reduced via our insurance contracts and agreements, we will discount our fees for those who are paying out-of-pocket. In addition, when fees are paid at the time of service, we offer a 20% discount.



Financing is available through the CareCredit® network, a healthcare financing program offered by GE Capital. For more information, please visit the CreditCare website.


Laboratory Tests

Lab tests (such as blood work, pap smears and other tests) ordered by your physician will be billed by the laboratory. We do utilize an outside lab, Laboratory Corporation of America [LabCorp], which performs blood draws on site in our Anderson office. This provides extra convenience to our patients, as well as quicker access to lab and test results. LabCorp has committed to price matching with every insurance carrier, so you will not pay any more for your testing through them than you would for services contracted through your insurer. Any questions you may have with these bills should be directed to LabCorp or the lab that performed the test.


Missed Appointments

If you cannot keep a scheduled appointment, please call at least 24 hours prior to that time to reschedule. You will be billed $25 for each missed appointment. The missed appointment fee for Diet Consultations and Urodynamics' Testing is $50 due to the fact that a significant amount of staff resources and time are reserved for these appointments. If you fail to show up without notification for two appointments and refuse to pay the "missed appointment fee," we will no longer be able to accept you as a patient. New patient appointments are booked for one hour creating a substantial gap in our office schedule if these visits are failed. In the event that a new patient fails their first appointment they will most likely not be allowed to reschedule, unless they can document extreme circumstances or pay a $50 missed appointment fee.


Returned Check Fee

Returned or "bounced" checks will be subject to a $40 fee. When a check is returned for non-sufficient fund (NSF), we are actually billed fees from the bank. In addition, extra costs are generated to collect the funds. Cash, credit card, or money-order will be required to pay the unpaid amount. Inability to obtain payment within 30 days will result in being turned over to collections.


Billing Questions

If you have questions regarding your account, please contact us at (765)646-8569 and ask for Jennifer in our billing department. If your account has been turned over to a collection agency or you have filed bankruptcy and included our practice in these proceedings, we will no longer be able to provide you medical services unless other arrangements have been made.

We understand the medical care can be expensive, and we will make all efforts to assist you in formulating a payment plan. However, like any business, we cannot survive and pay our bills unless we receive payment for services. This being the case, we adhere to the policy that if your account becomes past due, we will take action to recover the amount due within 90 days of initial billing. If a patient is turned over to collections, they are usually terminated from the practice.
